Description of Fields and Control Buttons in the "Add Survey" Form of the "Surveys" LMS Section

This modal window represents the "Add Survey" form used to create and publish a new survey or questionnaire among users in the "Surveys" module of the LMS system.

1. Language Selection Panel (Top Section)

Ability to enter the survey title and content in different languages:

• O'zbek (Uzbek): Section to enter the survey title in Uzbek (currently active).
• Русский (Russian): Section to enter the Russian version.
• English: Section to enter the English version.

2. Mandatory Fields (*)

• *Title / Name : Input field for the title or topic of the survey/questionnaire.
• *Start Date : Start date and time of the survey active period (formatted as dd.mm.yyyy hh:mm).
• *End Date : Closing date and time for participating in the survey (formatted as dd.mm.yyyy hh:mm).
• *Target Audience (For Whom) : Checkboxes to select which user roles the survey is intended for:
• Student
• Teacher
• Head of Department
• Dean
• Marketing

3. Action Buttons

• Save: Confirms the entered survey information, saves it to the system, and publishes it for the specified target audience.
• Cancel: Discards the entered information and closes the form.
• (Close): Close button for the modal window.
